Important News from the Nurse

Medical documents,  like physicals,  immunization records, and prescription medication orders may be dropped off, mailed, or faxed to the school nurse over the summer months.  All documents will be secure and confidential.  

I will be back in the office August 19, 20, 23, 25, and 31.  

ATHELETES - please make sure you get your Sports Physical (and ImPACT consent form, if you still need to turn one in) to the nurse or athletic Trainer.  DO NOT GIVE YOUR SPORTS PHYSICAL OR ImPACT CONSENT TO THE COACH

Please see your private reports on edline regarding the most recent sports physical on file in the nurse's office.
